Health Records Export for Maccabi

By installing or using Health Records Export for Maccabi ("the extension"), you agree to these terms. If you do not agree, do not use the extension, and remove it from Chrome. The extension is developed by Roni Rachmani ("the developer").

1. What the extension is

A free, open-source tool that saves a copy of your Maccabi Online records to your computer. It is unofficial: it is not made, reviewed, endorsed or sponsored by Maccabi Healthcare Services, and Maccabi Healthcare Services does not support it. "Maccabi" and "Maccabi Online" belong to their owner and are used only to describe what the extension works with.

3. The medical file order

Each export orders a fresh copy of your full medical file through Maccabi Online. The extension sends the same request the site's own order button sends, but asks for your whole history, which is a wider date range than the site's form offers. By starting an export, you ask the extension to place that order for you. Maccabi Healthcare Services sends you an SMS about it, and the new file replaces the previous one on the site. When a copy ordered earlier the same day over the same range is already waiting, the extension uses that one and orders nothing.

4. Not medical advice, and not a complete record

The extension copies what Maccabi Online provides and does not interpret it. It is not a medical device and does not give medical advice. An export may be incomplete or out of date: the site does not offer every record (for example, imaging studies, or visits older than 12 months), and some items can fail to download. Items that could not be exported are listed in the extension's popup when the export finishes. For medical decisions and official copies of your records, rely on Maccabi and your healthcare providers.

5. Your exported files

The ZIP file is yours and stays on your computer. You are responsible for keeping it secure, and for how it is used or shared after it is saved.

6. Availability

Maccabi Healthcare Services can change its website, or limit how it can be accessed, at any time, which may stop the extension from working, fully or in part. The developer has no obligation to update, fix or support the extension, and may change or discontinue it at any time.

7. Open-source license

The extension's source code is available under the MIT License. The license governs copying, modifying and distributing the code. These terms govern your use of the extension. The extension includes third-party components under their own licenses, listed in third-party-notices.txt, which also ships in the extension.

8. No warranty

The extension is provided "as is" and "as available", without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy and non-infringement, to the fullest extent permitted by law.

9. Limitation of liability

To the fullest extent permitted by law, the developer is not liable for any indirect, incidental, special, consequential or punitive damages, or for any loss of data, loss of access to your Maccabi Online account, or harm arising from your use of the extension, the exported files, or their disclosure. Nothing in these terms limits liability that cannot be limited by law.

10. Changes to these terms

Changes are published on this page, which comes with each version of the extension, and in the project's repository, with a new effective date at the top. When a change is material, the extension shows its notice again and asks you to accept the updated terms before you start another export. Otherwise, using the extension after a change means you accept the updated terms.

11. Governing law

These terms are governed by the laws of the State of Israel.
